Package Requirements

Casts, orthotics, and Rx order forms must be included in FedEx Box or KLM Brown Box (provided by KLM) airbill (shipping label) on box - Driver will bring a pre-printed FedEx label; if not, KLM provided you with preprinted aribills (no forms to fill out!) that have your name and address, KLM's address, and all other crucial data. Peel and stick bottom copy of shipping label to box. The top copy is your receipt.
